Why You Have to Trim the Wick

The mushroom cap, explained
A 'mushroom' is a small carbon cap that forms on the tip of an untrimmed or low-quality wick, caused by incomplete combustion of the wick material and any wick-strengthening additives. It sheds soot into the air and onto the jar as it grows and eventually falls off. Regular 1/4-inch trims before lighting prevent it almost entirely — it is a wick-maintenance issue, not a wax-quality issue.
The trimming routine
Extinguish the candle and let the wax fully cool and re-solidify. Using a wick trimmer (a curved-scissor tool designed to reach into a jar and catch the clipping) or clean scissors, cut the wick to roughly 1/4 inch above the wax surface for cotton, or about 1/8 inch for a wood wick. Clear away any trimmed wick debris or old carbon before relighting — debris left floating in the melt pool can itself become a secondary wick and burn unevenly.
